In order to get crushed by an elevator you need to call the elevator to a level above the ground floor then jump down and call the elevator while standing on the platform. The elevator will come down crushing you netting the achievement.

Amusingly, if you're on top of an elevator and get crushed by the ceiling and die, it doesn't count. You have to be under it. Some useless info for ya.

I never even thought of trying that. That is awesome though--shame the achievement didn't unlock.
