Go talk to Decker about Mitchell, and he'll tell you about him being in the Army. After that, go into Hank's shop in Chinatown, and he'll give some documents about Mitchell.
Once you're inside Redwood-Watts, you need to go left from the Foyer, into the Security area. You'll find a locked door that requires you to go back to the Foyer, take a lift up and enter the Administration zone. Hack the computer there, and you'll be able to acess the office at the end of the Security area.
Hack the computer on the right, and then go to the Garrison (two screens right from the Foyer). Enter the first door and take the lift up. Mitchell will be there. Talk to him, and you'll unlock the achievement,

Additional Info: This is essentially a two-part achievement. The first half you will need to do before going to Redwood-Watts and the second while you are there. Much like Crash's and Ellie's achievements/quests, you need to be mindful of what you do and in what order so as to not lock out their respected achievements.

When you are tasked with getting the three key cards, Mitchell the head of security will come up. Speak with Decker first to get the inside scoop on Mitchell and come to find out that you need to dig deeper into his past to find a way to exploit him. Decker lets it pass that Mitchell was ex-army opening to door for us to talk to Hank in the Chinese Quarter to find out more. When you speak with Hank about Mitchell he ends up giving you Mitchell's psych evaluation as well as video evidence of Mitchell's crimes. The next portion you do at Redwood-Watts.
Mitchell is in the barracks section of the facility (all the way to the right), but we want to use the footage Hank gave us in Crash's computer to broadcast the footage to the whole facility. In order to get to Crash's office head all the way to the left and hack the camera feed. Once you upload the footage you can head to the barracks and up to the top right where Mitchell's room is. Confront Mitchell and unlock the achievement. I highly recommend choosing the dialog option 'bye' instead of coaxing him on afterward. By choosing 'bye' you are able to earn quest experience and allow Ellie to deal with Mitchell (Ellie's quest line) where as egging him on will result in you killing him, missing out on 1,000 experience, and possibly blocking out Ellie's quest and achievement.
