Quest: Reach Fixer's Hope and then Adapt or Perish.
Provider: Raycast and then A.J.
Location: Dex's apartment and then outside of Eden in the Afterlight Boulevard.
Additional Info: Reaching Fixer's Hope is a main quest and fighting Lorraine is a necessary part of the main story. Adapt or Perish is a side quest that requires you to go back down to that original area where you will fight Lorraine a second time thus unlocking the achievement.
During the beginning of the game you drop down on Lorraine for your first combat experience which is more or less the combat tutorial. Whether you defeated her and her goons or not does not matter, but this represents encounter one.
When you reach the Afterlight Boulevard district you will run into A.J. who is standing outside the closed down restaurant Eden. After speaking with her she will suggest you meet her at the Nightingale bar to discuss things in a more private setting. Once you speak with A.J. at the bar the quest will start and she will direct you to BioFarmTech which is in the Highrise district. Speak with the receptionist and either pay $200 to get the information or convince Claire with the line about lack of women on the board. Claire gives up the name and address of an accountant who tragically died working with BioFarmTech. If you already completed the Sales Report and Unaccounted For quests you can give A.J. the information you've already collected and head right to the sewers.
From Fixer's Hope you will want to head down into the sewers and to the right toward Harbor Hall. You will back track down the original area where you came out of the Sewers reaching Fixer's Hope for the first time. As you move through the Sewers you will notice new enemies and amped up security that you can either get rid of or run through. When you make it back to the large open area you will notice none other than Lorraine is back and has some taunting words for you. This time around you need to defeat her and when you do you will unlock the achievement.
