For this achievement do you need the Horadric Cube, 6x Perfect Gems (One of Each Type) and 1 any Magic Amulet when you have this things then put the in the Horadric Cube and craft the Prismatic Amulet.

I hope that helps the new players who have not playing D2 :)

Magic Amulet is a blue one?
What will I receive after? A gold one? Any specific stats? Or it's just a status without any special bonuses?

.
.
Just FYI:
I put in a blue level 30 amulet with each of the different 6 perfect gems, on Nightmare difficulty, and the result was,

Blue, Level 31 Amulet,
+5 minimum damage,
All Resistances +16.

And achievement unlocked.

I'm trying now with a "Plauge Emblem Amulet" that's effects prior to fusion are;
Req lvl 15
10% chance to cast lvl 3 charged bolt when struck
+1 curses (Necro only)
+1 Strength
Replenish Life +9
+9 Maximun Stamina

It's not letting me, I've beaten the game on easy and am a level 41 Necromancer. I just beat the 4th quest in Act one and feel like it should have worked...

Switching Amulets to a lesser quality one we have the Amulet Of Absorption; I'm now level 42 and have completed all but the Andy quest for Act 1.
Req level 19
Damage reduced by 3

It worked! My first Amulet must have been to good. It was that tan/sandy gold coloring instead of blue. The results are;
Prismatic Amulet
Req level 31
All Resistances +19

I got that amulet by simply mixing rings in the Cube.

Would have been nice to know, that the old Amulet gets "destroyed" in the process. I was hoping to add the "All Resistances +19" to my Amulet. But you get a completely new one.

To address the first comment for anyone in the future: Magic items are the blue text non-named items. Yellow items with a special name are rare. You need to use a magic item for this recipe. Yes, it will destroy the original amulet, though most magic amulets are very low tier anyway, unless they have +skills on them.

6 Perfect Gems (1 of each type) + 1 Amulet (Magic) = Prismatic Amulet
Use this formula to create an Amulet with Resistance to all. ilvl = 50. The amulet will always have the Prismatic prefix (All Resistances +16-20) and has a chance at getting a suffix as well.
