This achievement is glitchy, but you can unlock it with any combination.
For example, I had 6 online softcore characters and an offline harcore paladin and unlocked the achievement, so you can match softcore and hardcore characters, online and offline characters without problem.
As long as you have one of each class kill baal, you are good.
If it doesn't unlock when you kill Baal with the last character, just repeat the kill with every character. Mine unlocked after repeating the kill with my Paladin. You don't have to kill it alone either, jump into a baal run game and just leech the kill if you want.

You might be screwed, it might work. The game is unreliable at saving the kill for the first time.
