How to unlock the Chaotic Whispers achievement in Diablo IV - Definitive Guide
7/29/23: Looks like this still works on Xbox after the patch that was supposed to fix it. Perhaps because of the difference in methods of tracking between D4 challenges and Xbox achievements?
Once you complete the campaign, you will unlock the Whispers of the Dead mechanic, which will target several areas of the map and spawn special objectives for a set period of time in that area, which will grant you a number of Grim Favors upon completion (generally 1, 3 or 5 based on the time the task requires).
After obtaining 10 Grim Favors, bring them to the Tree of Whispers in northern Hawezar (which you will find during the campaign). The tree will offer you three random equipment caches. The one you're looking for to unlock the achievement is specifically the Collection of Chaos. Upon selecting this cache on ten different turn ins, you will unlock this achievement.
Whether or not you are offered the Collection of Chaos on any turn in is random, you may go several turn ins without being offered one.
As mentioned in the comments, if you are going strictly for achievement efficiency and aren't concerned with Sacred/Ancestral drops or experience, the most efficient way to do this is likely on a high level character on WT1 as the mobs and dungeons only scale to 50.

Comments on the other guide implies the Collection of Chaos is almost impossible to get. In my case, I had one in every third-ish pick. RNG is RNG so every player will have their own experience with grinding this one.
Now, a quick guide on how to quickly getting 10 Grim Favors for a pick at the tree:
Every region will have an area with different tasks to perform, each having a value to some degree reflecting the effort invested. A small event will give you one Favor point, tasks getting you traveling across the zone will gave you three points while a dungeon will gave you five. I found it fast to complete a dungeonand then go for two small events while harvesting 100 souls (worth three points at the same time).
I found "Culling" tasks where you are asked to kill 50 specific monsters in a small area taking too long for the one point you get - Avoid these if you can't perform it while doing another task at the same time.
In the PVP zones, the Seething Abomination can be killed for five points and the blood shards from that kill can be cleansed for another three points at an Altar of Extraction. The PVP zones of course brings some risk of being disturbed by other players. For Hardcore players like myself doing these eight fast points brings a risk dying and ending it all but it seems Hardcore players are avoiding these zones. In my whole grind, I didn't meet a single player here.
Good luck grinding this one. When putting focus on completing this, the grind won't be too long.

These Whisper events, or Grim Favours, require you to perform a certain activity designated in the world until you’ve amassed 10 Grim Favours. Once you have you can head back to the Tree of Whispers and exchange those 10 Grim Favours for a reward that comes in the form of a cache.
Every so often one of these caches will be labelled as a “Cache of Chaos” i.e. a cache that usually consists of a bit of everything. Open 10 of those and you’ll unlock this achievement.
Certain activities in the world net you different amounts of Grim Favours. They usually work out this way:
- Complete a Whisper dungeon - 5 Grim Favours
- Harvest venous motes from enemies - 3 Grim Favours
- Invoke rituals at 4 different altars - 3 Grim Favours
- Find a corpse and defeat a special enemy - 1 Grim Favour
- Cull a certain amount of one enemy type (AKA Khargai Crags Bandits) - 1 Grim Favour
- Clear a smaller dungeon (like a cellar, etc) - 1 Grim Favour
- Complete a world event - 1 Grim Favour
There’s also some PVP events that can be designated Whisper events too:
- Defeat the Seething Abomination - 5 Grim Favours
- Complete a ritual at the Altar of Extraction - 3 Grim Favours
- Complete a PVP zone world event - 1 Grim Favour
Of course, for them to count, they must be marked a Whisper event.
A few tips on how to speed things up, a little:
- Where possible combine two events at once: i.e. with an area with both venous motes and an area where a certain enemy needs killing, go to the area to kill the specific enemy whilst also earning venous motes.
- World events are quick, as are the smaller dungeons. In and out with 1 Grim Favour in a matter of minutes.
- If you have side missions in specific dungeons, wait until one of them is a Whisper event before doing it.
- Always go for the 3 Grim Favour events i.e. 100 venous motes and the ritual ones are by far the best use of your time.
- I usually do: 1 dungeon, 1 venous motes collection, kill one lot of enemies and then do a world event and that’s my 10. Can usually take about 20-30 minutes to do the lot, depending on your build.
Once you’ve opened your 10th Cache of Chaos, you’ll get this achievement.

It is advisable to start it as early as possible from the moment you access the tree (at the end of the campaign) and combine it with other activities during the execution process (nightmare, heltide, events), since the probability of the necessary boxes falling out is extremely low.
