If you're only concerned with speed, you can just run straight through the three dungeon levels without killing anything (as long as your timer allows it).
You do not need to get any of the reward upgrades (filling the bar) for completion to count, just making it to and killing the final boss gives you credit. The timer stops when you enter the boss portal.

No, but you can rush it easy in 2-3min per run, I just got the achievement, and last 10 runs just rushed and defeated the boss
