I had a small area left in Five Hills and couldn't figure out how to get in there.
Turns out you have to complete two side quests to access this small area.
Quests found in the The Tangles area of Five Hills
The first one is 'Lost in the Jungle'.
The Second is 'Blood in the Grove'
The second mission takes you into this area and if like me if it is the last area to defog the achievement pops upon entering.
