I unlocked this during my first run boss fight by using two upgraded poison cards. One was just do d6 damage, but on a 6 inflict 3 poison. The other was do d6 damage, max 5, and inflict 1 poison. I just hit the boss with them each turn, using the warriors reroll ability to get a 6 each turn.
