How to unlock the Item Master achievement in Digimon Survive - Definitive Guide
This achievement is a huge grind and one that should be started from early on in your time with this game. Throughout the game, you will move to many different locations on the map. Some are story related, some are used to develop affinity and some seemingly have no point of interest icons. Every location you visit has the potential to have hidden items available to find. To get them what you must do is press RT to open your camera and look around for parts of the screen that start to glitch. If you inspect these glitches you will either find a shadow battle or an item. You need to do this until you have acquired 100 items total. If you visit a location twice in a row you can have items spawn both times as long as there's been some sort of development or new dialogue unlocked, meaning you can't just repeatedly enter the same area and pick up an item.
There's no point in the story where you can endlessly just roam around and pick up items so you will want to be contributing to this from the start or else you will have to do extra playthroughs of the game just so you can have new items spawn over and over again.
Thankfully, the number of items found carries over to new game plus so as long as you get in the habit of opening your camera whenever you enter a location you should be able to unlock this pretty naturally.
It took me roughly two full playthroughs of opening the camera every time I landed at a new location to unlock this.

Figure I'd just add you can get this in one playthrough. I got mine to pop at the free action in the beginning of part 11. Not that it really adds much because most people will probably do at least two playthroughs anyways.

I think that it is somehow tied to your profile and not the save file. I got mine to pop during the beginning of chapter 6.
I did restart the game twice and saved on a different save file each time. First time, I reset after chapter 2. Second time, I reset after chapter 3.

This achievement is unlocked during "Free Action" and "Exploration" parts. Always visit every location and check with your phone every corner. Aside from the training itens to help your team and some battles to level up, sometimes you will find a secret story text for one of three stories. When you get five for one of them, you will get a really strong ability.
The game tracks this stat over multiple save plays, so if you don't get it in your first run, load up your "cleared" save, which allows you to keep everything as it is other than affinity and Karma points.
The "quickest" way to do this is to keep visiting areas during the exploration and free roam sections of the game. Free roam everytime the screen goes black and everyone moves loacation, you can go back around and do it again.
You can press R2 to bring you phone up and then press R1 or L1 to move to the glitches it picks up or you can look for magnifying glasses without a name tag (i.e. "school") and then press R2 and scan it in with your phone.
Takuma will always say "Hm, what's this?" if you have found a hidden item.
