After the long adventure in the waterway you'll end up in a fight with Megaseadramon and some minions and a few turns in there will be a cutscene.

Just use your Vaccine types on him and you should be fine.

Weird, im past that point in the story and the trophy didnt unlock for me. didnt got Ultimate Ascension either. EDIT I was using remote play, when i didnt got the trophy had to reload a save and fight again now directly in the console, Trophy unlocked this time.

This achievement is Story Based and can not be missed. You will fight Wendigomon after a little battle with MegaSeadramon. Keep in mid that this fight can not be changed except during the True Route when playing at New Game +, since Shuuji can be saved with at least 30 affinity.
