Got this on my 2nd run. The reason it has the fewest completions of the companion cheevos, is that it has a pre-req that isn't as blatant as the others. The cheevo is unlocked in The Castle Hale map in Port Grimard and you will get the quest (My Name is Beth) after talking with Corisandre in Yllian only after you destroy the 2nd Mirror's Eye (first one is on the very first map) in Atellian. Right at the start of the map, right behind the Emperor, is a wooden fort with the Mirror's Eye in it; however you cannot interact with it until after you beat the map storyline and go back to the fort to pick up the mission (completing it gives you Corisandre's Legendary weapon as well).
