warning, this achievement is semi-fraudulent. you won't get it on a normal playthrough in any mode unless you own DLC. without DLC, it involves a lot of grinding the ever-same spot and is even missable. first of all, you will not get this achievement on your first playthrough, but on the second one, which is called liberation mode and has an xp boost. the first playthrough will leave you on ~lvl 67 if you do everything, liberation mode on ~lvl 75. difficulty or group composition has no influence on XP earned. if you don't own DLC, you can only go to a later area called "queens nest" and beat the "storm lord" (i think) over and over, because it is the only respawning enemy in the game - but only until you progressed in the area's quest line, which can/will (unsure) make him go away. the mob is max level but has only 3 enemies in it. it will grant 90 XP each time and will take 2-3 minutes. you will have to repeat it about 30-50 times though, as a level is always 1000xp, but you get less for everything the higher you climb. aside from that, make sure to do all quests and fight all mobs.

OK, a week after saying the game wasn't that good, I got the DLC on sale and tried Liberation Mode. And I will say Liberation Mode is both fun and you can hit Level 80 by just playing the DLC and not grinding.
Few points about my run:
- was playing in Liberation Mode ... much more fun
- was fairly complete: all quests, conquer all resources, un-fog all maps areas
- I did not pick fighting for every option. Some conversations have friendly options as well as combat options ... I just played "normally" (i.e., letting people live who didn't harass me)
By the time I got through the Queen's Nest area, this achievement popped. This is the last map before you enter endgame ... meaning, I did both the Barat Nor and Path of Flesh and Flames maps/quests before tackling the Queen's Nest.
