How to unlock the Ghostly achievement in Dishonored 2 - Definitive Guide

Typical stealth achievement, gained for getting through an entire mission without a guard spotting you. When the guards start to see you, the stealth indicator for them will start to turn white, turning red when they've spotted you. As long as it never turns red, you'll get the achievement.

Standard practice is to save often and reload if unsure. Easily obtainable on the very first mission.

Obtained by completing one level without being noticed.
If you are noticed, then load the checkpoint or the last save and try again.
Same as it says in the trophy tooltip Clean hands , check your statistics in the first two tabs, stealth mode should be 0 (zero) detections.

You can kill, the point of achieving this is so that they don’t see you and raise the alarm
