Once you get to mission 5, pretty much just follow the waypoints. You can go stealthy or a mad dash. Either way, make your way to the Eye of the Dead God (getting Dead Eye in the process). Once the cutscene ends, turn around and you'll be in the void. Go over to the telescope on the cliff and interact with it to see a new waypoint leading to the Outsider.
If you're careful, you can make your way down the cliff using a combination of falling and Displacement/Blink (if playing Original Game +). Once at the bottom, go through the portal (at the waypoint), and then follow the last waypoint to the Outsider. You'll have two options. Pick the first, and kill the Outsider and get the achievement.

Unless you did the little sidequest to gain the option to spare the Outsider, killing him will be the only option at the end of the game. Otherwise, you'll get to choose.

This achievement is obtained in Mission 5, A Hole in the World.
After interacting with the Eye of the Dead God, you will find yourself in the Void. Turn around, and on your left, near the cliff edge, trapped within Void spikes, is a dead cultist. Approach his body, read the note on his corpse, and loot his key. You will now receive a new optional objective marker on your HUD, leading you to Malchiodi's lodgings. Next, turn around and use the telescope in order to mark the Ritual Hold's location on your HUD.
From here, follow the waypoint that leads to Malchiodi's room. When you arrive, open the door and approach the safe. Use the combination of 962 to open the safe and then loot/read the contents within. Now you can make your way to the Ritual Hold. When you enter through, you will find yourself deeper within the Void. Continue to follow the waypoint on your HUD. Eventually, you will encounter the souls of dead men, including Daud, and be just a few feet away from the Outsider. Speak to Daud first, then interact with the Outsider. You will be presented with two options: Kill or Spare.
In order to unlock "Deicide," choose to kill the Outsider, which involves Billie thrusting the Twin-bladed knife into his heart. The final cutscene will then begin to play and the achievement will unlock.

Soon enough, Daud will agree to spare the Outsider, approach the target, and whisper his name into his ear, freeing him of his curse. The final cutscene will then begin to play and the achievement will unlock.
