Chapter 2

Before you can use a Fountain Pen as a weapon you have to buy the upgrade in the Black Market.

After you got the upgrade, go back upstairs from the black market and into the building above it.

One of the rooms on the second floor has the Fountain Pen next to the bed. Pick it up and use it on an enemy to get the "Mightier Than The Sword" achievement.

Has to be a guard I think , tried it on the sisters in level 4 .... they were all I had left alive by the time I found a pen as I was going for the contract to kill everyone.

Strange. I did this on an unconscious guard, reloaded the save and did it on a standing guard (insta-killed him) and the achievement didn't pop. This was immediately after I picked up the fountain pen. It eventually popped randomly after I shot a wolfhound in the head with it.

For some reason this didn't pop for me on my first try, but awesomely the guard I killed was in the little guard station directly outside the apartment building in the video. Well on the little desk in there was another pen, so I was able to use that on the guard that was killed in the video and it popped that time.

I did this on Chapter 3 during the bank heist having already purchased the Makeshift Bolt upgrade (500 coins) from the Black Market

Having decided to go through the roof and use the poppy tincture (available to purchase/steal from the Black Market or win at the auction across the street) to knock everyone out asleep, I found a fountain pen on the desk at the southern end of the eastern room on the top floor of the bank. I shot the Voltaic Gun at a sofa in there to empty it, picked up the fountain pen, and shot a sleeping guard in the head.

Simple!

Partway through the game you will find an upgrade at the Black Market that allows you to turn objects into weapons. There are plenty of pens in the bank.

This achievement is slightly non-obvious. Once in the second mission, make your way to the Black Market. You need to purchase an upgrade that allows you to create basic bolts out of everyday objects. It is on the second page of the upgrade purchase list, in the second row. Once you have that you can find a fountain pen in the apartment building directly above the black market, next to the bed. To make things simpler and guarantee you're using the pen, save your game, fire all of your bolts, then grab the fountain pen so that it is the only bolt you have (it's not clear if this step is necessary, but it was the only way I got it to unlock and guarantees your only shot is the pen). Head back down and find a guard without a helmet (there's likely one across the street and to the right of the front entrance with his back to you if you haven't eliminated him already). Go up to him (if you're too far away it probably won't work) and shoot him in the head. If you kill him with the shot, the achievement should unlock.

This achievement can be obtained as early as Mission 2, provided you have at least 500 coins.

During Chapter 2, Follow the Ink, locate the Black Market in town and speak with the merchant. You'll want to buy the Makeshift Bolt upgrade (500 coin) for the Voltaic Gun. This upgrade will allow you to use ordinary items, such as a fountain pen, as ammunition.

After purchasing the upgrade, exit the Black Market and enter the apartment building directly above on street level. On the third floor of this building complex, enter the closed apartment door and turn left once inside. You can find a fountain pen on a bookshelf directly above the bed. Now you can equip the Voltaic Gun, return to the streets, and execute a headshot on any of the guards. Just make sure that the fountain pen is equip on the wrist gun before attempting this, otherwise you'll just shoot a regular bolt.

Once you have shot the guard in the head with the fountain pen, the achievement will unlock.

We do it in the 2nd mission. We go to the black market and buy an upgrade for the gun, which allows you to use improvised trash as a projectile (500 coins). After this, you will be able to collect pens and other trash. We find, make sure that there is a handle in the thrower’s chute and give a headshot to the guard. I'm not sure if the guard should be without a helmet, just in case I shot at the guard in blue uniform.

Got it in the third mission - it turned out to be enough to shoot a stunned guard with a pen in the head - the helmet didn’t help

If you're wearing a helmet, you need to hit it in the face. Otherwise, the helmet flies off and is not given.

Almost above the entrance to the black market in the second mission there is a ladder bridge on the street. We move to it, go into the building on the third floor and there in the apartment there is a pen on a shelf near the alarm clock. Then, from the same room, we go out onto the balcony and shoot from above in the head of any guard on the street, for example, the one who reports the carriage accident.
