You will get this in Mission 3 (The Bank Job). Once you get inside the vault, there are 6 safes. Starting from the left, enter the following codes:
1) 011
2) 235
3) 813
4) (This is the safe with the two keys from the previous mission. Open this to get the Twin-bladed Knife achievement.)
5) 455
6) 891
As soon as you finish putting in the combinations the safes will all open and the achievement will pop.

You can complete this achievement during Mission 3: The Bank Job. It comes near the end of the mission, once you've cracked into the vault. You can enter the vault either by getting above the vault from the top floor and disengaging its brakes or by talking your way into the security office and moving the vault to Dolores Michaels's office.
If you've thoroughly explored the bank, you found evidence that the bank employees have been trying to crack Jindosh's backdoor into the safes. On the second floor, you'll find a note indicating that there is a pattern to the digits that you need to enter. The pattern starts 0,1,1,2,3,5,8,13. This is called a Fibonacci sequence, and each number is generated from the sum of the two numbers before it.
There's a catch, however. You need to skip over the double-key lock in which the Void Blade can be found while continuing the pattern. Thus, the sequence to open the safes is:
1) 011
2) 235
3) 813
4) Skip: Key lock. (The next numbers in the sequence after 13 are 21, 34. This safe would thus have the numbers 213 if it did not require keys.)
5) 455 (This safe gets the 4 from 34. The next number in the sequence is 21+34=55.)
6) 891 (The next two numbers in the sequence are 34+55=89 and 55+89=144.)

2022 its bugged theres no way you can get this , i even tried NG+ and it wont pop also the codes has changed

Not bugged. If you put in the Fibonacci sequence into all the safes, they will then open all at once and will unlock the achievement. Opening them individually will not unlock the achievement.

This achievement seem to be bugged, as it do not pop even after unlocking every safe (5 with codes, 1 with keys). It do seem like they have changed the codes too, as it apparently used to be using different codes.
Codes is as follows, starting af nr 1 (skipping the fourth safe, as it uses the keys)
