Chapter 2
Go to the upper floor of the Specor Club in Chapter 2. You can find lots of Plagued Spirit bottle there that you can instantly use on the 3 people sitting next to each other to get this achievement.

In case anybody has the same issue I had, you may need to clear the system cache for this to unlock. I did what the achievement required about 15 times and it just wouldn't unlock. I cleared the xbox cache and it unlocked when I loaded back up the game.

Another place this achievement can be easily obtained is also in Chapter 2 with the Mime (who has an optional contract).
Accompanying the mime are always two civilians. Simply grab a bottle of Plagued Spirit from the Spector Club and walk through the alley (past the taxidermist) to the spot with the Mime.
When I popped this, I used Displace to get up to the large pipes on the street opposite where the Mime is performing. Once there, quicksave (so you can reload if necessary) then hurl the bottle so that it lands directly in front of the Mime and it will impact both him and the two civilians.

I got this trophy inadvertently by being kinda stupidly completionist. At the Albarca Baths, I'd already destroyed all the bottles in the crates upstairs, but for some reason I thought I oughtta destroy the ones at the bar too (I didn't have to; I'd already gotten the checkmark for the contract.) Anyway, the moment I picked up one of the bottles, every Eyeless in the club instantly went attack mode; the gang sliced me up within seconds. As I fell to the floor dead, I got the trophy. One of the NPCs must've broke the bottle open when attacking me and they all got a faceful of it. So that's one way to do it easily

This achievement can be obtained in Mission 2, Follow the Ink.
After gaining access to the Spector Club, head past the bar and take the flight of stairs in the back to the upper floor. Once you reach the upper floor, pass through the open doorway. You should see three Eyeless NPCs lounging in chairs directly ahead. At this point, make a manual backup save.
Turn to your right and you will find two bottles of Plagued Spirit on a coffee table. Grab a bottle and throw it at the three NPCs in the chairs. They will stand up and almost immediately begin vomiting, thus unlocking the achievement. Unfortunately, this causes the Eyeless gang members in the club to become hostile, so make sure to reload your previously made backup save and carry on with your playthrough.

Grab a bottle of Plagued Spirit and then find a group of people. Chuck the bottle at em and make sure it breaks and watch as they all start to hurl their guts out.

You can do this on the very first mission while you're in the fight club. Right at the corner of fighting ring (in the ring itself) in a bottle of Plagued Spirit you can pick up (it's amber colored with a dead rat in it). To unlock the achievement you need to throw it at a group of 3 people clustered moderately close together. One easy way to do this is to aggro them so they'll gather to fight you then toss the bottle at them. If the cloud gets three of them the achievement will unlock. Just save before hand so you can try again if you miss and/or you don't actually want them mad at you as you play through the game.
