This is achieved during Mission 05. The specific setting for this is when you meet the outsiders real body within ritual hold.
1. Go up to teh outsider and interact with him
2. Select the top option which goes along the line of 'I haven't changed my mind'
3. interact with the outsider again after the cutscene plays. This is when you will trigger another cutscene and show you stabbing the outsider.
4. Cheevo popped.
