In Mission 5 you will encounter Envisioned Cultists once you enter the void.
I found that my weapons weren't really doing a whole lot, and getting too close lead to my death as they are very strong. Their projectiles also seem to be able to pull you toward them, so be careful.
I got this achievement by using Foresight to mark an Envisioned Cultist (the first one you encounter after entering the void), then placing a Displace marker at a point I knew it would stop (the golden circles). Once it was in place I displaced into it and that seemed to stun it, allowing me to assassinate it with my sword.

When you first enter the Void, one will come to investigate your entry. If you climb back up into the alcove you dropped down from in the real world, you can watch it approach. I just stuck a grenade to its back and then dropped down to kill it when he knelt down in pain.

Just adding onto this, I find hyperbaric grenades can work well for this Strategy. (you gotta cook it a little so you can down the cultist quicker rather than just throwing the grenade at it and waiting for it to explode.)

Envisioned are the large rock-like monsters who appear in the final part of the last mission. Apparently they are very hard to kill but there is a incredibly easy way to take them out.
So to kill an envisioned: first hit it with a (almost) fully charged Voltaic gun shot. It will drop to its knee and you can run straight up to it and assassinate it. You can take them out in a couple of second like this.
The charged Voltaic shot is an upgrade which can be bought at any black market shop (there are no shops in the final mission so be prepared).

While in the void, set a trap. I found the best place is in that first hallway right near where you come in.
I used two lethal set hook mines, one on either side, and a spring razor mine in the middle. I shot flaming arrows at it to get its attention, and then when it was stunned I shot at it a few more times and then went in for the kill with my sword.
Be very careful! It will instantly kill you if it gets to you! So keep it at bay as best you can. It's minions are easy to kill should it spawn them, but in true video game fashion they will disappear as soon as the big bad is dead.
I would save before trying this, as it can be tricky.

Another way, that I find easier, is that you first use Foresight to see where the Envisioned is going to stand, and put a Displace marker there. You then have to make sure you are in range, and use Displace to go where the Envisioned is at and it will stun them. Quickly use R2 to kill it. Mind that it could take a couple of tries as it might notice you. Doing this where you first enter the Void is a good place, though you might have to run if the Envisioned teleports to where you are at.
