How to unlock the An Unfortunate Accident achievement in Dishonored Definitive Edition - Definitive Guide
When you head down to the steam room to eliminate Morgan Pendleton you will see a room to the left as you head down the stairs. Go into this room and you will find a steam pipe and a Crank Wheel, pick up the wheel and place it on the pipe and turn it. This will pump steam into the room with Morgan Pendleton inside and net you this achievement.
He isn't always in the steam room for everyone. You can take him down with a sleeping dart and relocate his unconscious body into the steam room and use the valve.

There's reports of Morgan Pendleton just poppin up in the steam room which makes this shit ez asf. However if he isn't there and you are sick and tired of waiting for his sorry ass to get down there, you can follow this video where you use some 'convincing tactics' to attain this ez.
You can get the Pendleton's location from the ledger in the Madam's office once you are inside the Golden Cat. She'll also have the master key you need to get into the small room next to the steam room which will allow you to turn the crank and kill Morgan with steam. If he doesn't spawn in the steam room you can sleep dart him, drop him in the room, close the door and activate the steam
WARNING: Morgan will not always spawn in the Steam Room!
You can only do this Trophy on Mission 3 (Pendleton Twins).
1. Once Morgan has spawned in the Steam Room, first you need to grab the Master Key from The Madam. Once you have the key, and come back to where Madam and a Guard were talking, head to the double doors and turn left to see a little opening for you to mantle.
1.1 You’ll see a List of Rooms made in Gold next to a door. Head through that door and in the middle of the room, you should see 2 Women of Culture hugging or whatever they are doing. You should be able to see a set of stairs heading down. Now head downstairs where you’ll hear 2 Guards talking about their pay. If you look left, you should see a metal gate/door.
2. Once you’re in the door, you’ll see a circular window. If you look straight down, you’ll see some pipes with something that’s missing. Look Right and you will see the Crank Wheel and some Copper Wire. Grab the Crank Wheel (or both) and plop it into the missing section. Whenever you get bored of Morgan and the Woman of Culture talking or still being alive. Turn the Crank as hard as you can and watch as they panic to death.
