How to unlock the Clean Hands achievement in Dishonored Definitive Edition - Definitive Guide

I recommend trying to get this achievement while also going for the Ghost Achievement. This should also coincide with the Poetic Justice achievement and the Just Dark Enough achievement. After each level, the box at the bottom of the Mission Summary screen should be checked where it says that no one was killed. The "Hostiles Killed" and "Civilians Killed" should also read 0 at the Mission Summary screen.

Beware of accidental deaths. This is most likely to occur in Mission 7, where some enemies are near the edges of large heights. Incapacitating them could result in them just falling off and dying, which could void the achievement. A good way to counter this is to pay attention where enemies land, move bodies, and saving often.

During my Ghost/ Clean Hands playthrough, I did not actively look for collectibles and skipped most of the side quests, such as the Granny Rags side missions. The only side missions you should do are ones that are directly involved in using the nonlethal means of eliminating the targets. The fewer interactions you have, the smaller your chances are of having anyone die by your hand.

Since you will not be killing anyone, all targets must be eliminated by indirect (nonlethal) means. All potential lethal interactions should be avoided also. This includes all of the following actions:

The only means of taking out enemies (besides targets) were a standard nonlethal takedown (holding cn_RB) and sleep darts from the crossbow.

The second solution from BIG L RETURNS states:
CONFIRMED: Using mission select will VOID this achievement. This has to be done on one brand new playthrough. If you killed someone on accident, reload the save before you made your kill, if you cant remember or don't know where it was you'll have to just restart the entire mission over.

I'm assuming this is the same for the Definitive Edition as well. Sorry, you're going to have to start over if you definitely want to have the achievement unlock.

I'd just like to pop in and say that the Granny Rags glitch seems to have been fixed. I burned her cameo, hid, and tranqued her before she saw me, and I got this achievement and both the stealth ones.

Something I would like to add, I did the first mission(Escaping the prison) and when I got to the end someone had died and I was able to restart/replay the mission by the option it gives you on the stats screen and still get this achievement.

Do I have to start a completely new game, or just go to the mission select tab and play through each mission without killing anyone, because that's what I did, and now I'm nervous I have to start all over for skipping the prologue.

I made sure each mission had 0 kills, got every other low chaos achievement. Did all the alternative eliminations. Didn't pop with everything else at the end.

Started a new playthrough, only did the non lethal options for every target, stricly used only sleep darts or choked people unconscious. Every mission has a stats page and each and every one has the mark checked for "Didn't kill anyone". The achievement is still locked for some reason.

I've played through the game three times now and this achievement hasn't popped. Every mission had a tick on "kill no-one" and "never detected" but it still hasn't popped.

Still no unlock after trying 5 times, i'm going to delete the game, reinstall it, play it again with no kills, if then it doesnt unlock, i dont know wtf to do, i did legit everything.

UPDATE:
I deleted the game, installed it fresh, clean run yet again no achievement popped, i seriously am surprised.

UPDATE #2:
When you have to go to the sewer at the Pub you will see for the first time 2 creeps, zombies or whatever they are called. USE SLEEPING DART ON THEM, and dont let them fall in the water, they count as kills if you kill them and will lock the achievements Ghost and Clean Hands :)

Completed the game with each mission showing no kills or alerts and no achievement. Why am I not suprised that something isn't working in a Bethesda game

Didn't unlock on my first or second playthrough apparently to the decade old Granny Rags bug. Instead of starting a 3rd playthrough to avoid doing Granny's missions, I reloaded a save I made while standing right next to her and Slackjaw in the sewers in mission 7. What I did was grab the key and kept spamming Bend Time until I reached the exit for the level. I had just enough Elixirs to restore my mana enough to keep spamming it. I was fast enough so that even the objective to help Slackjaw didn't pop up. This must've done the trick as once I saved Emily and the stats screen appeared both Ghost and Clean Hands popped. Hope this helps.
Imagine my shock that a Bethesda game still has unpatched bugs from over 10 years ago.

The following solution will give you 5 achievements (Ghost, Shadow, Clean Hands, Just Dark Enough and Mostly Flesh and Steel) worth 260 GS. During the playthrough you will get some other achievements as well but you will find out when doing so.
During my second playthrough I made a huge mistake but only found out when I finished the game for the second time.
I had to do it a third time. That made me to make this solution so it will save you a lot of time.

The youtube link will give you a perfect guideline on how to go through all the levels undetected and without any kills and only using Blink.
All credits go to Red Dragon who is responsible for this perfect walkthrough.
In his videos he only does the main objectives so you don't have trouble of "Granny Rags". I read a lot of comments about Granny Rags on the internet and the trouble she gives of not killing and staying undetected but in this walkthrough this problem won't be there.

To get all these achievements you have to do it in one playthrough, no mission select option.
Off course you can save during the playthrough and I recommend to do that quite often.

There was just one big issue for me wich let me made another playthrough so keep this in mind when you get to the following point.
Between mission 2 (High Overseer Campbell) and mission 3 you'll be asked to go in to the sewer to check what's going on.
Now take your Sleep Dart Crossbow and go in to the sewers.
Once you see the 2 weepers sleep dart them. If you kill them it will not give you the achievement at the end of the game.
Also be aware that when you sleep dart them they don't fall in the water because that still count as a kill.....!
During the missions when you have to choke somebody be aware to lay him on a save spot so the rats cannot eat him and not on a roof of wich they can fell of. I also lay them on the ground with X instead of throwing with RT just to be sure you don't send them to the here after by throwing them.

Last thing I would like to mention is that if you are going for the Mostly Flesh and Steel achievement as well, don't upgrade Blink because when you, do you will not get the achievement.

With this in mind you can follow the link bellow to get all these achievements.

Followed this guide, yet I never unlocked Ghost, Shadow or Clean Hands. I checked the mission results screen at the end of each level, and I was credited as such. Had exactly the same issue when I played this on 360 on my old profile. Same 3 achievements didn't unlock, but I tried 4 times back on 360 to no avail. Not sure if I can muster up the patience again.

Otherwise great guide and a thumbs up from me.

Sorry to hear it didn't work for you. If I remembered well you have to do it in a playtrough and not mission select. Assuming you did it in a playtrough it's strange the achievements didn't unlock...facepalm
