Very early in the game, after you wake up in prison and retrieve the explosive, you will soon enter a room with a waypoint that says "Door Control". There are three guards here. Quickly activate the door control and run into the next room that opens to lure the two guards there after you.
Note: there are also two guards in the room prior to this that you can alert instead if that is easier.
Either way, once all five guards are in combat with you, simply kill them and, achievement unlocked.

If you missed it here like me you can get this at the next mission along with the wall of light kill. Just rewire it and get into combat with 5 or more guards.

An easy way to get this done is in Mission 08: The Loyalists.
1. Just run around the pub and get a whole gang following you.
2. Bend time
3. Murda them however you want.
4. Cheevo popped.

You can do this on any level.
Tip: Make sure you have plenty of Health Potions while you are doing this.
The easiest level to do this is Lady Boyle’s Last Party.
The Trophy only popped for me when I was done fighting everyone and not just after killing 5 people.
1. Once you are in the main building, kill the Overseer with the Music Box first. Then run around or just get lucky that multiple new enemies show up to fight alongside the others like me. Just keep killing everyone that challenges to fight you.
You don’t have to just use your sword, but you would be pretty cool.
Tip: Make sure you have plenty of Health Potions while you are doing this.
