All you need to do is purchase the Possession Power posses a rat and run through a broken vent.

Perfect spot in Galvani's house on the 2nd floor. If you've been collecting and/or saving runes you should be able to get it here.

This achievement is bugged for me (same as the Big Boy one) - I’ve done it multiple times, even got the one for staying in a vent/in possession for 3 minutes, and this hasn’t unlocked yet.
With Big Boy, it eventually popped after 7 or 8 assassinations with just my sword, so I’m hoping that eventually this unlocks too after doing it enough. I’m probably at around 12 to 15 uses of it so far from doing my high chaos run.

Simply learn Possession level 1 and possess a rat and then walk into one of the small rectangular rat tunnels. Walk inside of the tunnel a bit (took me a few seconds) and the trophy will pop.
