Work towards this achievement on your low chaos playthrough.
Simply do not alert any enemies. Each enemy has an indicator above there heads, if any of these turn white it is ok this just means they are suspicious, so just hide until these disappear however if this bar then turns red then they have been alerted.
It is best to make saves every few minutes, the moment an enemy is alerted just load up your last save. At the end of each mission you are informed how many enemies were alerted if anything other 0 appears then you must replay the whole mission.
Just to note it does not matter if enemies find bodies.

Just thought I would throw this in there; having alarms rung is not the same as alerting someone. I completed the 2nd mission without killing or alerting anyone but had 3 alarms rung but still got the achievement for not alerting anyone.

Ok, question.......I'm doing a no kills/no alerts/no powers/low chaos (obviously, lol) run, I'm trying to get as much cheevos done in one playthrough.....thing is that I did both Granny Rags & Slackjaw's side missions.......am I going to be screwed in mission 7? I heard there is a gate that becomes locked if you did both side missions and you have to steal the key from Granny Rags.....

The guide I'm posting will not only get you Ghost, it will get you completing the game in Low Chaos, NO kills at all, no upgrades at all and not alerting anyone.
Make sure the 2 boxes are ticked off at the end of each mission.
Save often in multiple save locations and make a save at the beginning of each level in case you don't get the boxes checked.
Also make sure you do everything as he does. In the second mission I think, I accidentally talked to the wrong person triggering a side quest, and when I failed that, it triggered an alarm so I had to redo the mission. It's at the very start of the mission before Samuel takes you to the city. If a girl talks to you at the top of the stairs, load the last checkpoint. ( actually there is a way to do it this way, but it's not covered in the video)
Posting the first video, the rest are in the side bar. He goes through the game really fast and I liked his commentary, you may not.
