This achievement requires you to complete the first mission of the game without being spotted.
Taking your time with this mission is crucial for this achievement. In addition, you can utilize manual saves to help you. Once you clear a section or a spot that might be troubling you, quickly make a manual save. This way, you won't have to restart the entire mission or revert to back an autosave if you fail.
Tips if you are struggling:
- Make use of manual saves, aka saving after each section so you won't have to revert back to mission start or an autosave
- Avoid enemies when you can. However, if you kill/neutralize any enemy, make sure to hide the body
- Stick to corners, walls and darkness
- It is good to look through keyholes before using a door
- Always remain crouching as you will make less noise

Save after every room that you pass quietly. You need to be very careful to keep an eye on the guards and make sure they are not looking at you. A white mark or more is fine, but it's better to avoid them all together. White marks quickly lead to red ones. The hardest part of the level not to be seen in is after you set off the bomb. The explosion isn't as big as you might think, so stand on the stairs and quickly run straight off the end of the walkway into the water. Stay there a moment and move toward the shore. Get out of the water into the reeds and enter the door. If the guards say something like "I know you're here somewhere" rather than "if he makes it to the sewers" you have succeeded. The next difficult part is after you get to the area with the hole in the floor and the guard talking about Weepers. Take him down, lethally or not, and enter the water in this area. You can surface under some of the bridges for air, some of them are not high enough. Once you get to the area with the fallen beam, get out of the water. You are safe from here to the end of the level.
