How to unlock the Back Home achievement in Dishonored - Definitive Guide
The most conveniently placed overseer is in mission 2, when rescuing Martin. He's the only guard there and even when he calls for help nobody comes. You can take as much time you want to.
If you do not have bend time or can't afford it at this point it is still possible. But remember that granny rags has 3 runes for you at the start of the mission. This will make it easier. Stand a good distance away from him after alerting him. He has a never ending supply of grenades. Catch with X throw with RT.

Great solution. This is the best place to get this chieve for the simple fact that it is one-on-one.

SPOILERS AHEAD
Grenades are primarily carried by the Overseers (the monk-soldiers wearing the metal masks) and thus primarily encountered in Chapters 2 (High Overseer Campbell) and 9 (The Light at the End) On chapter 9 I did it near the entrance on the right from the boat* - disable the Arc Tower, go up the stairs and aggro the guard spawn. Kill guards for awhile and an Overseer will eventually run out.
It helps tremendously if you have Stop Time 1 or 2 (2 is much better). Find an Overseer, aggro him and kite him around until you hear him shout 'You will burn!'. This usually indicates he is about to toss a grenade. Pop stop time and grab it out of the air. The red semicircle indicating the fuse will then start to fill. When it fills the grenade will explode. Time it so it explodes _in the air_ near him when you throw it back at him. I spent a considerable amount of time dropping it by his feet etc and not getting the achievement.
When it kills him from a thrown mid-air explosion, the achievement will pop.

What do you mean by "kite him around"?

I mean to keep backing away from him so that he won't always try to slash you - eventually he will hop back and throw a grenade, which he likely will not do if he is focused on hitting you with his sword.

Video-guide of Dishonored to get the achievement Back Home. For this near the end of mission 2 we'll find an enemy only with the skill of throwing grenades. using 2 runes we'll learn Blend Time skill. Use this skill when he throws it to catch the bomb and give it back, and we'll have the achievement.
As we all know mission 2 is the best place to do this preferably at the start with martin but if you miss that opportunity this video shows you how easy it is to get this achievement regardless,
Mission 2
There is an overseer 1/3 of the way through, startle him, then throw his grenade back at him.

Occasionally Overseers (the masked guys) will throw grenades at you, which you can pick up with and throw back with like a traditional item you can pickup and throw. You may want Time Bend for this to make it a bit easier. The achievement can be a little glitchy, so try and kill the guy by getting a direct hit with the grenade, which seems to work 100% of the time.
In mission 2 is a great place to do this. After entering Holger Square you have to help a trapped guy named "High Overseer Martin" (cannot be missed). Right in front of him is a normal Overseer. Just make some noise and run around until he throws a grenade at you.
