As the achievement describes, you must start from the first mission and play till you get to Kaldwins bridge killing less than 10 people. You will get this if your playing stealth and or following the guide for clean hands and ghost. Sersphim17 Wikigameguides does a bang up covering these achievements as well as the entire game in stealth.

Pops once you carry Sokolov to the boat at the end of mission 4 as long as you have killed less than 10 people over the course of these 4 levels. Guaranteed to pop on a Clean Hands play through.
