How to unlock the Off Track achievement in Disney Epic Mickey 2: The Power of Two - Definitive Guide
In order to get this achievement you must NOT do any of the train station side quests. Essentially on your first play through, you're going to want to only do the main quest and thin out everything you possibly can. This will prevent you from getting the 'Perfectionist' achievement which you'll want to focus on in a different play through.
The other thing about this achievement, is that it can only be acquired AFTER you've beaten the game. Once you've beaten the game, you have to go and visit the four train conductors and upon visiting the last one the achievement will pop. Here is a list of conductor locations.
1. Mean Street South - In a booth right up against your initial starting position. When facing out, the conductor is on your back right.
2. OsTown - When entering OsTown, the conductors booth will be immediately to the left between the two gates.
3. Bog Easy - This train conductor is NOT in a booth like the other three are. He's out in the open. What you'll want to do is enter the store, then walk through it, and exit on the other side. You'll then continue until you reach the "Jug Band" (made of the three alligators) and take a right. You'll cross a bridge and the conductor will be the cow standing there.
4. Ventureland - The train station is in between a tiki hut and the TV Station that daisy is in (a small building with a pink roof, and small circular yellow window, and a daisy door mat) on the left. The train station itself will have is a building on the left where the tower has collapsed. When looking straight at the building, the booth is on the left of the building.
Once you have talked to all these conductors the achievement should pop.
Also just as a time saver if you have the red tickets you can use the Cinema to fast travel to OsTown, Bog Easy, and Ventureland; otherwise you'll have to use the projectors in Mean Street North.
Note: While I have not experienced the similar glitch MitoticActivity said,"When I went back to Ostown after finishing the storyline the conductor was not there. I was a bit nervous that I had messed up, but went to the station at MS South (already went to Bog Easy and Ventureland) and got the achievement." If this happens to anybody else please confirm in the comment below.

Okay, I did not have the same experience, but I'll make a note of it in case others are experiencing the same glitch.

Do not do any of the train station related quests, this will cause the quakes to eventually destroy all of the stations earning you the achievement - however you may be prevented from earning the 'Perfectionist' achievement.

So you have to play the whole story a minumum of two times to get both the achievements?

Yeah it's a bit stupid - it's not just this one that's missable either. There's a quest that you'll miss to get 'Running the Gauntlet' but won't be able to do perfectionist. Check my x360a guide/roadmap for details

The other guides are great, walkthru too, but if you're like me and have left the game for a while, remembering what to do drags a 3 hour playthru out substantially. No credit to me at all, all credit towards this video, give them a thumbs up.
After completing the game you can simply visit OsTown, Ventureland, Bog Easy through the fast travel hub, and head over to Mean Street South via the balloon.
