How to unlock the Bonus Bank achievement in Disney Infinity - Definitive Guide

To get this achievement, you need to land on 5 bonus tiles while doing spins for vault items.
When you load up a page of stuff to spin for or refresh your current page, 2 of the tiles will have yellow sunbursts behind them. These are the bonus tiles.
To get them, you'll need luck and/or spins as using a spin will randomly select a tile on the board to award to you. Fortunately, after winning an item, the tile becomes blank and can't be selected again unless you refresh the board, making things easier to get the more you spin. Depending on your luck, it will take anywhere between 5 and 47 spins to get 5 bonus tiles.
One other small thing to note, it is possible to miss this achievement, but you really have to be trying. What you have to do is manage to deplete the vault so much that it runs out of items (That takes over 100 spins to do so). If there are less than 17 items left to unlock, then the bonus item spots will no longer appear. To prevent that, have at least 16 spins when you go to do this, and don't leave the screen or shuffle until you have both bonus items.

Every time you open up the Vault there will be two items marked with a gold star, these are the bonus items.
In order to GUARANTEE you get them, you will need 16 spins to unlock EVERY space on the Vault page.
If you're lucky, you might get one of the two bonus items first.

Once in the Disney Infinity Vault, you will have a screen with 16 random items, 2 of the items on this page will have a yellow star background. These are the bonuses. Since where you land once you use a spin is random, you can guarantee the 2 bonuses on a page by waiting until you have 16 spins to start. Once an item is received it won't be replaced until you shuffle so eventually you will be guaranteed the bonus items using this method.
