You get medals from completing objectives on tour races (currently there is the Starter Tour and the Season Tour), generally each race has three that can be earned. Complete 150 objectives to earn 150 medals and the achivement.

Oi tem que comprar algum passe pra desbloquear as conquista,ou ta bugado no xbox pois ja tenho pelo menos 50 medalhas e nada de desbloquear?

You should earn medals by completing seasonal and limited events as well.
