The third character will be located somewhere between zones 2-4. The character will be identified on the map as a yellow heart with a heartbeat symbol. Get to the person, who will have a bunch of yellow orbs floating around them, examine them, and then click them again to bring up the option to use an adrenaline shot on them.
A word of warning though. Do not examine them until you have an adrenaline shot with you! Once you examine them, the death countdown actually starts. Up until that point, you aren't on the clock to get them revived.
