If you don’t have an undead in your party, you can dismiss one of your companions and hire a mercenary from Sergeant Zrilla back at your ship (the Lady Vengeance). Ask for an arcane warrior > witch and you’ll get an undead fighter that can make the trip.
You can also survive the trip by having a Glowing idol of rebirth, but surviving the trip that way doesn’t pop the achievement. You only get it by using an undead.

The ferryman's pier is located in the north of the Monastery Forest. The lake is covered with the fog of death, only the undead will survive on that shore, for this achievement they must be on the team, if they are not, then we run to the ship, drive out any Persian and hire the witch Taylia from Sergeant Zrilla.

For those playing in co-op:
The trophy will be given to the one who crosses the river. In our case, the first player (host) played as a gnome, and the second as Fain (undead), so the issue was resolved simply by the second player leaving the game, which gives the first player control of all the characters.
