Miss-Able: No (Main Story Quest)

Once in the source temple you can explore the initial area for progress in some quest lines, particularly near the broken mirror.

Afterwards, go to the northern path in the cave and find a large boulder/cave-in. Locate a small switch to left of it on the wall to make it disappear. Continue onward through the new tunnel to find a new waypoint shrine.

The puzzle presented by this room is entirely optional, you can skip it.

Walk to the furthest north room, it contains a circle of candles on the ground.

To open the door and unlock the achievement do the following.
1. Light each of the outside candles, wait a few moments for a lightning strike.
2. Light only the centermost candle, wait a few moments for a lightning strike.
3. Light the West, Center, and East candle.

To begin obtaining this trophy, you need to get through the stone doors in the Temple of the Source itself. These doors will only open when you have restored the entire tapestry at the end of time.
To do this, you will need to collect and use all the star stones and bloodstones. After use, inert stones remain. Don't forget to pick them up from the ground so you know how much you've already collected. There should be 16 stones in total. Here are their locations:

1) In Teliron’s hospital, with the doctor’s assistant Evelyn, in Saisil.
2) At the scene of Jake's murder at the King Crab Tavern in Cyseal.
3) In the Black Bay cave, in the chest of Pontius the Pirate.
4) On the road to Silverglen, near the portal guarded by the Fire Wanderer, in the Cysila location.
5)Under the desecrated church of Cysila, in the lair of King Barakk.
6)In the treasury of the winter king Boreas in Hiberheim.
7)In the talking iron maiden, in Boreas prison.
8)In the Temple of the Immaculate during the test in Silverglen.
9 and 10)Under the Chapel of the Immaculates in Silverglen.
11) In the Temple of the Immaculates during the rite of human sacrifice, in the Forest of Luculla.
12)In Moradino's office, which is in his secret lair near the goblin village, in the north of the Forest of Luculla.
13)In the totem of the goblin village in the Forest of Luculla.
14) During the quest “War for the Stones” in the Knight’s tomb, near the settlement of Hunter’s Wasteland.

After collecting and using all the stones, go to the time weaver at the end of time. After talking with her, the doors will open and you can
go through the maze for this trophy.

I would like to add. Theoretically, 11 stones should be enough. This can be observed at the end of time through open portals. There should be 11 open. Except for the restricted area. She doesn't count . I collected 12 stones and the door to the temple opened. Despite the fact that I found the 12th one after opening the door.
