As a note, you actually have to take them from the chest for the achievement. If you open the chest but leave then in, you won't get the achievement.

Once you get to Cyseal, you'll eventually be given a quest to investigate a murder. This investigation will eventually take you to Esmerelda's shop. Take the stairs up to the 2nd floor. You'll find the underwear for this achievement up here...I believe it was in the chest. You'll get the achievement as soon as you pick it up.
