When you reach the area past the infected log in the Phantom Forest, you will find Casandra in the far North corner. There are many ways to approach this part, but it is very important that you DO NOT burn her bones, as that will make you miss this achievement. The best course of action is to go through the Temple of the Dead and reach the waypoint, then go kill Balberith and obtain the Stasis Fern. Then you can go to Casandra and get this achievement, then reload and burn her bones and return to get the other one.
