Crossfades are done using the crossfader (when you switch to the green or blue record only). I recommend playing at least on Medium to get some crossfades here and there. If you can play Expert, play on this difficulty since you will get a lot more crossfades.
You should get this achievement by just playing (if you are going for all achievements for this game). If you still need crossfades after this or you just want this achievement, play "Galvanize" on Expert; it contains many crossfades.

I don't really understand so do I switch back and forth or keep on one side during the fade

Performing rapid crossfades during freestyle sections works great! Got this achievement on the second day I played the game as a result of this technique (by accident).
Slamma
This will come with time depending on which difficulty you play. The higher the difficulty, the more crossfades there will be. Playing through Empire Mode will get you there, if not, very close.

This will come with time. If you play on Expert, then you can actually achieve this in one complete Empire Mode playthrough. Otherwise, since you'll need to play a crap ton of online to get the full 1000, this will be unmissable.
