Following the excellent guide will help big time, but it's worth noting that you must go right up to the door way. I must have done 4 or 5 clean runs before thinking it was glitched, and then it popped right as I got to the entrance.

MISSABLE!
You need to get through the entire Dalek Temple level without being seen. I've posted the relevant part from my walkthrough below.
On the low wall in front of you are three Dalek eye stalks. Pick them up.
Notice that on the other side of the wall is another button.
Select one of the eye stalks, hold L2 to aim it, and throw it (R2) as far away from the button as possible
When the Dalek is moving, walk (DON'T RUN) over to the button, click it, and go into the door nearby
In the next room, pick up the Dalek head/top and throw it onto the button that opens the next door – Bullseye!

Pick up one eye stalk and the Dalek head/top in this room before leaving
Be careful - this Dalek will occasionally look in the direction of the door you're in.
When the Dalek isn't looking, sneak around the right side of the room, but don't pass the last pillar
When the Dalek isn't looking, throw an eye stalk into the corner across from you
When the Dalek is moving, click the button and go into the door
There are three eye stalks on a low wall in this final room, if you need them. The Dalek can see you, though, so be careful
Get behind the pillar, but far enough away that you can see the Dalek. This time, you're going to have to throw an eye stalk on the button. It's possible to hit it from the entrance door, but it's tricky to get the position right. If you're trying from the door, use the Dalek head/top from earlier to give yourself the best shot at hitting the button – aim it just above the button and it should activate the button. Just be careful of the patrolling Dalek. Leave the room when you can – the trophy all this is tied to will pop soon.
Once outside, pick up the journal entry on the left before the bridge (27/41)
Cross the bridge and SLOWLY enter the new room – Can't exterminate what you can't see
will unlock now if you got through the Dalek temple without getting caught. If it didn't pop here, close the game and try again. If you go too quickly into this new room, you'll hit the level end trigger and will either have to reload your back up or play through a new game for another shot.
