A new game is required to get the achievement if missed as there is no way to replay chapters or go back to the area.
Once the back door unlocks to the office, the safe will be to your left. Input the code 1483 and the achievement will pop immediately.

Out of curiosity, where is the code found in the room? I think 14 was in the back of the photo frame, but I didn't see 84.
