Seems this achievement is a little picky. You need to press 
at the right time when a ranged attack is about to hit you. I could not get this to pop reflecting the poison blobs the bugs shoot, but I was able to get this with the zombie enemies that shoot fireballs. I reflected probably 10-15 poison blobs and got nothing, but first time I reflected a zombie fireball the zombie died and I got this achievement. Not sure it needs to die, but maybe the reflected attack needs to hit the enemy. For the bugs, I couldn't get the reflected blobs to hit the bugs.
