To get a multi-kill in Doom you will need kill three or more enemies with the shotgun in one shot. Here is a method to set it up in E1M3, repeat twice more for the achievement. You may also just get this naturally through gameplay but for those who want it ASAP here is a video for you!

Popped a second ago after I killed TWO enemies with a shotgun for the 3rd time.

It's not killing 3 or more with one shot. Its 2 or more. Just do it three times.
But I guess you're not wrong... three or more would pop it as well

This can be done on any level but as the other guide says E1M3 is the easiest, as the shotgun is handed to you at the beginning.
After that just try to line up the two gunners in the right corridor and take them both out.
Note: Contrary to what the other guide says, you only need to kill two enemies at a time to get a Multi-Kill, so this is probably going to come up early just through regular gameplay.

The easiest way to get this is play on the default difficulty and episode 1. There will be several clusters of pistol zombies that will die from one shot since they are close together.
