How to unlock the Lost Collector achievement in DOOM 3: BFG Edition - Definitive Guide
There are 9 PDAs in the "Lost Mission" campaign. I found a video that shows the exact location of each and also the lockers and the RAGE room. Credits go to MrReign420!
There are only 9 pdas in lost mission and most of them are in plain sight . Xx Grim R3ap3r over at xbo360achivements wrote a good solution showing where they are all along with pictures.

This campaign is short and for the most part, linear. The PDAs are very easy to find even without a guide. There are only 9 to locate across the 8 levels and the only other achievement associated with this campaign is the Rage logo.
Below are the PDA locations along with the codes for the lockers you will come across on your way in the order they appear. I would suggest creating a separate save every time you find one of these just in case you miss one, that way you only have to go back as far as the last one you found.
I included the locker combinations simply for sake of convenience and for chronological reference. They are in no way necessary for this or any other achievement.
1. Enpro Sector 1
Allen Rhodes
Control room. At the end of level after dropping down though a window. In the same room where you listen to the doctor talk and take a disc. Not story related-missable
2. Enpro Sector 2
First locker/Shotgun 731
Chris Vargas
Coolant Control Room. Towards the beginning of the level after first seeing some lost souls. Same room as the first locker. Story related.
David Voss
Main Control Room. Almost right after the last one; take the elevator down and enter a small room with two computers one of which is needed to open the doors in the large room. Right on the computers. Not story related but very hard to miss.
3. Underground 1
Locker 054: 631
Bob Thompson
Filtration Monitoring Fairly early in the level, after heading down the ladder. You will come with a lift in the middle. When you push the switch to call the lift, many ticks/spiders will attack. The PDA is in a side room here before heading down. Not story related-missable.
4. Underground (part 2)the save file will not show it but it is a different stage

Central Sub-System Controll After finding the sentry and facing the two rocket shooting guys, it will be on a computer terminal before entering the room Sub-Level Corridor Sl-2 Not story related-missable

Exis Entry Hallway Really quick in this level. Enter this hallway and turn right and should be looking out a large window. Look to the bottom of the stairs. Not story related-missable

Exis Service Tunnel After Talking to the scientist you will go through a few more rooms until you come to a ledge with the Plasma gun. On the same ledge but behind the boxes. You will fight your first Bruiser here too. Right to the right of locker 071. Not story related-missable
6. Exis Labs (part 2) again not labeled as such but it is a different stage

Enviornmental Elevator Lobby After going through a narrow passage and seeing a man fall down an elevator shaft. Head to see it next to a fat version of Forest Gump. Not story related-missable

Production Coordination Office This is right after the locker and is just a smaller room in Exis Central Operations. Not story related-missable
7. Hell no lockers no PDA
8. Hell Outpost no lockers no PDA
Also, the final level, Hell Outpost, has an achievement too. Create a few different save for this level. After a long section of running down a narrow path with imps and Hell Demons, the game will do an auto-save. As you enter a large open area with blocks all around, turn to the left and find the Rage logo on a wall behind a pile of stones. Press the action button on the logo itself to open a passage and that is it for miss-able achievements for this campaign. If you reach the ground floor before, where the teleporter is, then you missed it. Head back to the autosave or an earlier save.

Just like all the other PDA collectable achievements, make sure you search each and every single place you can and make sure you kill friendlies (if any) as they may drop a PDA.
