In Hell Outpost of the Lost Mission campaign. Near the end of the level right before you drop down into the boos battle turn to your left and look behind the blocks and find the rage symbol press RT and open up the chamber. Achievement unlocked

At the very end of the Lost Mission campaign you will be walking though Hell. After you have gone through all the hellish looking castles you will be out in the open. Here you will see some stairs in front of you that lead down to a set of stone obelisks. Don't go down here as that triggers the boss, instead take a left and go behind the stone bricks. There will be the the RAGE logo on the wall behind these bricks. Simply interact with it and the trophy will unlock.

This one is a lot like the secret id Software room achievement. The location of the RAGE logo is behind a wall to the left of the final boss stage in the Lost Mission (it's when you walk into the area and you are on the second story).
