Very start of Delta Labs 4, proceed down the halls and you will come across two chainsaw enemies. After you dispatch of the second one look around for a skeleton leaning on the ground at a corner. Walk up to the skeleton and look at it for several seconds.

Got this completely by accident, I didn't even look at the damn thing. I didn't know it even existed until it popped for me and I had to look at the solution

Right at the beginning of the Delta Labs 4 level, go through the first bloody hall, take a left, go through the next two doors, and then in the far back right corner you will see a bloody skeleton and beside him these exact words will be written in blood. Shine your flashlight on them and this one will be yours.
