Worked a charm, I think I was missing ammo pick ups off of enemies when I tried it myself.

Great tips all round but if you prefer a visual of what to do, where to go, how to cheat for a lovely 100% then i have you covered :)
Following this guide should also net you the indiscriminate head hunter achievement too providing you are killing EVERYTHING that you come across!
Please note you don't NEED health to be on dead on 100%, it can be more which is fine.
Just make sure to pick up EVERY item, that includes enemies ammo..
So it would be good to just let off several shotgun blasts in order to pick up said ammo as it can be easy to just go past it without realising
