The description says: Kill all monsters in any monster-filled level on 'Hurt me plenty' or higher. But the achievement only pops up if you choose the 'Hurt me plenty' difficulty.

Load up E2M8 in Doom 1 on 'Hurt me plenty' or higher, then beat the Cyberdemon. Lost Souls don't count towards the kill count, so the achievement should unlock.
The level has a rocket launcher and plenty of ammo, so have fun!

You can get it, by simply killing every monster on E1M1 (Doom 1)

Does not work for me. Tried multiple levels in Doom 1 & 2, with + without cheats and with getting all items & secrets. No achievement.
Edit: The achievement was unlocked for me on the next day. So it was just a late / silent unlock in my case.
