When you return to the Fortress of Doom after the first level, you will automatically be directed to pick up the Sentinel Crystal after getting the Flame Belch to receive your first upgrade.

To achieve this you need to find 1 guardian crystal (the blue one). If available, select any module that pumps up health, armor or ammunition.

Get the achievement if you follow the story clues, as the first crystal is found when you first return to Doomhold.
