You need to press A- Actually I had a dream about printers .
Save the health powerups till you have one bar left if possible to maximise effect. I found the shotgun was better for the larger battles. You have to empty your magazine to reload to always do that after each encounter.
The responses in order you need to make are:
B - DC - Fan Boy
Street Fighter - Not Missable Level 1
B - Quit - Will Work For Coffee - (Restarts game)
A- Suck it up - A Quarter Mile at a Time
Either(Level isn't Missable) - No Respect
A - Try to sneak in(B skips level) - Lost Track of time
Either - Put the money in the bag
A- Actually I had a dream about printers - Down on a farm
A Unfortunately I already ate(B skips) - Movie Star
B - Umm I wonder what that would be like - There's Always room for zombies
Either - Fastest Gun in the west
Either - Mountain Man
A - Sounds Fun - Look at the brain on Doug
After Boss battle - Comeuppance

The biggest issue I had with this achievement was that the controls were very poorly explained.
is shoot
switch between gun and shotgun
reload
use health power up/grenade (not sure if you can switch between the two)
cover (I didn't use this)
For the first wave I used the rifle, same for the second. I switched to the shotgun for wave 3. Wave 4 I used my grenade. I took the left route when given the choice. The last wave can be tough but keep an eye on your health and use your health power ups when you are down to 2 bars.
Empty your clip and reload between waves to ensure you start with a full clip.
It's annoying but once you have the controls down it shouldn't take more than a few tries
