Hard mode is unlocked after beating the game once. For more information on Hard Mode, see Step 3 in the Roadmap.

The final area before the boss. The chaos is everywhere, as enemies from all areas hover around aimlessly. There are only one or two caverns, and no safe spots, making your descend harder than before. On the other hand, it's a great place to get those combo and no-landing trophies. Just keep in mind that, if you go for the latter, you can't enter any cavern, which is not recommended right now, if you haven't finished the game on Hard before.

As there are tons of stomp-able enemies, you will be recharging your gunboots frequently. Beware of the red ones though, as they can't be stomped and will charge after you (specially the flying skulls and piranhas). Careful, as the prices are the highest and you could have no gems to buy health if you run low on it. Try to get as many as health bars and increments to your clip as you can (like 25+ hit combos), then you can build up your hit points (7 is good, while 8-9 is ideal).

The trophy will unlock at the upgrade selection screen of Area 4-3.
