Once done speak with him again and choose the option "Can you improve my equipment?", and you should get an upgrade. If not there is one in the boxes in the forge. Go to the modify armour anvil, choose the piece you want to upgrade, choose the upgrade and then confirm the changes
Shortly after the tutorial is completed, you will be given a quest that includes going to speak with the smith. On your way there, do not sell your starter armor, for once you arrive, there will be a loot chest beside the smith (a crate) that includes an upgrade for said armor. Pick it up, then turn and face the two men striking anvils. As you just acquired an armor upgrade, face the anvil that says “Modify Armor” and press to activate the upgrade station. Follow the prompts to complete your first upgrade.
Alternatively, you can first complete the steps for "Well-Prepared" and then upgrade that, as it is the same piece as your starter armor.

Just like crafting weapons and armor, enchanting or upgrading is done at the Blacksmith. Select either the modify armor or modify weapons option. This will show you a list of all the weapons/armor you have which have upgrade and/or enchant slots.
If you look at the weapon/armor stats on the right, below the stats you'll usually see a few empty icons with some text next to it. Let's take standard Medium Armor for rogues for example. It might have 2 empty slots called "Medium Arms" and "Medium Legs". If you select the armor, you'll see the 2 empty slots. Click on either of them and you'll be able to insert an upgrade -if you have them- such as Arms that add extra health for example, or legs that add extra defense. Click on any of the upgrades and press to apply the changes. Each armor type has their own upgrades. Same as with weapons. Not all weapons and armor have upgrade slots available though.
You can acquire upgrades in a few different ways if you don't have any. Upgrades can be crafted, just like weapons and armor. You can find armor/weapons with upgrades throughout the game. These upgrades can be removed and used on other armor by going to a modify armor/weapons table. And they can be found as loot and bought at certain merchants.
